What this app is
This application is provided by Hooray Health LLC to help members of Hooray Health MVP plans find in-network care, review plan benefits, and access telemedicine. It is an informational tool. It is not medical advice, and it does not replace your plan documents. In an emergency, call 911.
Information we collect
This app does not require an account and does not ask for your name, member ID, Social Security number, or health information.
Location. If you tap Current Location, your device shares your approximate coordinates with our servers for the sole purpose of sorting nearby providers. Location is used for that search and is not stored or used to track you. You can always search by city or ZIP instead.
Usage and reliability data. We collect aggregate, anonymous usage statistics (such as page views) and technical error reports (such as a failed search) to keep the app working well. These reports do not include your identity.
On-device preferences. Your language choice and favorited providers are stored only on your device and are not sent to us.
What we do not do
We do not sell your information. We do not use advertising trackers. We do not share information with third parties except the service providers that host this application.
Provider directory
Provider information (locations, hours, phone numbers) is updated regularly but can change without notice. Call ahead to confirm hours, availability, and network participation before you visit.
